Healthy Start Care Coordinators assess mother’s needs and provide referral to risk-appropriate services, assist with Medicaid application, provide emotional support and encouragement, and deliver pregnancy and childcare information. Care Coordination is contracted through Charlotte Behavioral Health Care, Inc.
